Your NDIS plan is the paper the NDIA approves. It holds your goals and your funding. A provider's support plan is separate and holds no funding.

What is an NDIS care plan?
Most people say "care plan" when they mean their NDIS plan. This is the document the NDIA approves for you. It lists your goals, your funding and how your funding is managed.
Only this plan holds your NDIS funding. Other plans, like a provider's support plan, do not.
What is the difference between an NDIS plan, a support plan and a service agreement?
Your NDIS plan comes from the NDIA. A support plan is written by a provider and shows how they will deliver your day to day support. A service agreement is a contract between you and a provider.
A support plan and a service agreement do not change your funding. Only your NDIS plan does.
What is inside an NDIS plan?
Your plan has your goals, your total funding and your plan dates. The funding is split into core supports, capacity building and capital. Some supports are stated, which means the money is only for that support. Others are flexible.

How can my NDIS plan be managed?
There are three ways: NDIA-managed, plan-managed and self-managed. With NDIA-managed, you can only use registered providers. With plan-managed, a plan manager pays your invoices. With self-managed, you pay them yourself.

How long does an NDIS plan last?
Plan length is different for each person. Your plan shows its start and end dates. Before the end date, the NDIA will review your plan.
Check your dates now and write them in your calendar.
Can I change my NDIS plan?
Yes. If your health, home or support needs change, you can ask for a plan change. Send your latest reports from your OT, doctor or other health worker.
Do this early. Do not wait for your plan to end.
What if I run out of funds before my plan ends?
Tell your support coordinator or plan manager straight away. Do not wait. Check how your funding has been used, and ask for a plan review if your needs are higher than your budget.
Funding does not top up by itself, so a review is the way to ask for more.

How do I get ready for my plan meeting?
Follow these four steps. First, write your goals in plain words. Second, gather current reports that show what help you need every day. Third, look at how you used your current plan. Fourth, bring someone you trust to the meeting.
Keep it simple. Say what a normal day looks like and where you need help.
What is changing for NDIS plans in 2026?
The NDIA is changing how plans and reviews work over the next few years. Dates and rules can move, so always check the NDIS website for the latest.
If your plan is coming up for review, ask your support coordinator what applies to you.
Who can write my NDIS plan?
The NDIA approves your plan. A provider cannot write it for you, and Caring Hearts does not write NDIS plans. We can help you understand your plan and your goals.
A registered NDIS provider must follow national safety rules, and your support workers are screened and trained.
What support can I get from Caring Hearts if it is in my plan?
If your plan includes it, we provide Supported Independent Living, onsite support in an SDA home, accommodation such as respite, and 24/7 care for people with complex needs. We work in Melbourne and Perth.

Can I get the NDIS if I need aged care?
The NDIS is for people with a permanent disability who are generally under 65 when they apply. Aged care is a different system for older people. Some people can choose which fits best.
Talk to your doctor or support coordinator about which one suits your situation.
